I have added my capcha code with PhpFormMail to develop a small “request for quote” or “feedback form” module for any small site. Our Quantumcloud programmer Mr. Raju helped out with mail_config.php file. He separated configuration from PhpFormMail and put in mail_config.php. In order to use this module with your site, please download attached feedback-form.zip file and edit feedback.php to apply css and mail_config.php to configure php mailing option.
